Hackensack Meridian Health Earns Highest Level of NCQA Case Management Accreditation

Hackensack Meridian Health is pleased to announce that the National Committee for Quality Assurance (NCQA) has awarded Hackensack Meridian Health Partners (HMHP), Hackensack Meridian Health’s clinically integrated network, its highest level of case management accreditation status. HMHP is the only organization in New Jersey and one of 49 in the nation to receive NCQA’s three-year accreditation.

HMHP received the designation for the Braven Health Case Management Program – scoring 99.3 out of a possible 100 points. The three-year accreditation is in recognition of the program’s strong performance demonstrating the rigorous standards set forth in the NCQA guidelines.

They are always refining their techniques to ensure our patients receive the most current evidence-based care - the highest quality of care possible.” 

Braven Health Case Management is an innovative program which improves the health of communities while making health care more affordable through innovative collaborations. The program helps ensure quality care is delivered to the members of the Braven HealthSM – the first and only New Jersey Medicare company jointly owned and operated by New Jersey’s largest and most trusted health insurer, Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ) and two of the state’s leading health systems, Hackensack Meridian Health and RWJBarnabas Health. The program offers support, guidance and valuable information to help patients manage complex medical situations, which includes post-discharge contact and pre-organ transplant.
