Yes, you’re still free to visit any doctor that accepts Medicare. No one can (or will) force you to see a particular physician.
No. ACO participation is the responsibility of your doctor. You don’t have to do anything at all.
If your doctor is a member of the HMH ACO, you can use the link above to see all of the HMH ACO participating providers. The physicians will also have information in their offices to share with you. You can also contact the ACO for more information.
Your details will be treated with the utmost security and confidentiality. Your doctor’s office will happily tell you about their particular data protection methods.
Participating providers will be able to share key medical information. But you can opt-out of this if you prefer. You can also choose to have your name and any other information removed at any time by calling the Medicare number below.
In addition, Medicare won’t share info about treatment for alcohol or substance abuse without written permission from the person in question.
