Jane H. Booker Family Health Center provides a wide range of outpatient health care services to meet the needs of every member of your family. We understand that life is busy, which is why we provide numerous services in one convenient location. Our team works closely with specialists at Jersey Shore University Medical Center, ensuring well-coordinated specialty care when needed. Social workers are also available for assistance and accessing community resources, as well as Spanish/English/French Creole interpreters on staff.

HIV/STD Care and Testing
Testing Hours: Monday - Friday, 8:00 a.m. to 4:30 p.m. (no appointment necessary)

The Infectious Disease Clinic at the Jane H. Booker Family Health Center provides confidential STD and HIV testing, treatment, and management. We offer a specialized team of clinicians to help diagnose and manage these conditions, as well as dedicated counselors and a support team to provide resources and emotional support when needed.
HIV Testing and Treatment Services:
- Free, discreet and confidential rapid HIV testing
- Dispensation of medications on site, eliminating the need to visit a pharmacy. Initiation of medication can begin at our location until patients are on a stable regimen.
- PrEP (pre-exposure prophylaxis) for patients at risk of contracting HIV, significantly reducing the risk.
- Facilitation of referrals to appropriate medical and community-based care.
- Family care management for accessing appropriate health care services, including home care and hospice.
- Access to drug trials, investigational pharmaceutical programs, and low- or no-cost pharmaceutical programs.
- Gynecological and dental services on-site for HIV-positive clients.
- Access to behavioral health professionals for assessments and referrals.
STD/STI Testing and Treatment Services:
- Free*, discreet and confidential STD/STI testing and treatment for infections including Chlamydia, Gonorrhea, and Syphilis. (*The first test is typically free for Monmouth County residents only; however, please contact us to confirm costs and eligibility criteria before testing, as subsequent tests may incur charges. Additionally, there is a nominal fee for those opting to provide urine samples instead of blood samples.)
- On-site medication dispensing for infections based on test results, with easy follow-up scheduling. Many patients can receive their medications on the same day of their testing.

Our cardiology specialists provide comprehensive assessment for diagnosis and treatment of common cardiovascular conditions such as hypertension and heart disease.
Dentistry
Our full-service dental clinic provides preventative maintenance, including check ups, cleanings, and x-rays, as well as emergency dental care when needed.
Endocrinology
We specialize in the treatment of all endocrine disorders, including thyroid disorders and diabetes, as well as general endocrinology and metabolism problems.
Gastroenterology
We offer comprehensive care for a full range of conditions that affect the digestive tract, liver, biliary tract, and pancreas, such as digestive cancers, gallbladder conditions and swallowing disorders.
Gynecology
We offer compassionate care for women of all ages and provide services including pap smear, birth control, urogynecology care, menopausal education and more.
Hypertension Clinic
Our Center offers comprehensive care for complicated cases of hypertension, led by a board-certified specialist who provides multidisciplinary care through diet, exercise, and medication strategies.
Laboratory (Walk-In and Appointments)
Our on-site laboratory provides added convenience for the collection of blood, urine or other testing requested by providers.
Nephrology
Our experienced nephrologists diagnose and treat common kidney disorders in adults and children, such as chronic kidney disease.
Nutritional Counseling
Our registered dietitians help families live a healthier lifestyle through customized nutrition plans and support.
Prenatal and Postpartum Obstetrics
We help new and expecting mothers get the quality care, support and education they need from their first appointment until after birth, including high-risk patients.
Primary Care (Adult and Pediatrics)
We provide primary care services, both well and sick visits, for the entire family, as well as chronic disease management (including diabetes and hypertension), pre-surgical care, immunizations, health education and management, cancer screenings and more. When needed, rapid flu and strep tests are offered for added convenience.
Psychiatry
Our specialists can help improve individual cognitive and emotional well-being and specialize in common behavioral conditions such as depression and anxiety.
Pulmonology
Our pulmonologists provide expert care for conditions such as asthma and COPD and are equipped with the necessary technology for pulmonary function testing and other diagnostic tests.
Rheumatology
Our rheumatologists provide comprehensive care for management of conditions such as rheumatoid arthritis and systemic lupus erythematosus.
Specialty Surgery
Our team specializes in a wide variety of surgical procedures, including colorectal surgery, colonoscopies, robotic surgery, bariatric surgery, breast surgery, and surgery for skin cancer.
Trauma and General Surgery
Jersey Shore University Medical Center is a Level I Adult (and a Level II Pediatric) Trauma Center and provides patients with a comprehensive range of surgical services. Adult patients who receive surgical care at the academic medical center may schedule their surgical follow up appointments with specialists at the Family Health Center.
Urology
We offer specialized care for conditions affecting the bladder and urinary tract, ranging from kidney stones to urinary tract infections to cancer.
Eligibility & Appointment Information
If you have Medicaid, Medicare, Medicaid Managed Care, charity care or you do not have insurance, please call the main number to be directed to the correct appointment desk. An appointment must be made in order to be seen in any of our services. If you are a member of a Medicaid Managed Care HMO, the primary care physician must be a doctor within the Family Health Center for you to be eligible for primary care services. There is a charge for all services, however, if you do not have insurance, you can set up an appointment or interview with a financial counselor to complete a financial evaluation at 732-902-7080. Having the following documents will greatly assist in the financial evaluation process:- Proof of income (three months of pay stubs)
- Proof of address (driver’s license or envelope showing your current address)
- Social Security card
- Any type of insurance card
Checking and savings account statements
A complete evaluation of the financial evaluation and determination process will be provided during your interview. While in the process of applying for Charity Care, patients can schedule an appointment.Medicare & Charity Care Patients: The New Jersey Department of Human Services has instituted an insurance program called “Family Care.” You may become part of an HMO.
