As part of the new regulations, each patient has the right, with some restrictions, to:

• Review his or her own medical record;

• Request an amendment or correction to the medical record;

• Add supplemental information to the record;

• Restrict use and disclosure of your medical information;

• Authorize formal consent before health information is released other than for treatment, payment, or as part of health care operations; and

• Know who requested and received medical information for other than treatment, payment, or health care operations.

As part of our commitment to protecting your information, Hackensack Meridian Health and its workforce are prohibited by law from releasing your health information to any party not involved in your treatment or our healthcare operations. This restriction includes family members. An exception is made only when you provide written consent.

The "Authorization for Release of Information" form is the document used to grant HMH permission to release your information to a specific individual or entity that you designate.

How to Request Access

For requests to access your medical records, please fill out our authorization form, and mail or fax it directly to the hospital where you received care. If you would like to follow up on a request that you sent, please give our Release of Information Vendor, Datavant, a call at 1-800-367-1500.
