Who We Are

Hackensack Meridian (HMH) Patient and Family Advisory Councils (PFAC) consists of patients and family members who have received care at any HMH facility or are a part of the greater community. PFAC members bring their experience in partnering with the Office of Patient Experience, health care professionals, and leaders in sharing their unique perspectives. HMH PFACs are an instrumental component in supporting Hackensack Meridian with its number one priority, which is keeping patient centered care and family needs at the forefront. The PFACs have collaborated on various initiatives, projects and feedback sessions to help enhance the patient and family experience and much more.

What does a PFAC Do?

Members will serve as the voice of patients and families.  Members may offer feedback, information and recommendations on planning, policies, and procedures. Information from this group will provide leadership with an enhanced understanding of how to improve quality, program development, service excellence, communications, patient safety, facility design, patient and family education, staff orientation and education and patient and family satisfaction.

The PFAC Mission and Purpose

Mission

To support the Hackensack Meridian mission by providing resources and guidance to strengthen collaboration between caregivers and patients, and to assure the delivery of the highest standard of comprehensive and compassionate, patient and family-centered, health care throughout  Hackensack Meridian.

Purpose

To engage patients and their families in the work of the Patient and Family Advisory Council, thus having a direct impact and influence upon care and services within Hackensack Meridian. To assure alignment, integration, and continuous quality improvement in patient and family centered care.  The Patient and Family Advisory Council serves as a formal mechanism for involving patients and families in policy and program decision-making.

Members will be expected to

  • Commit to serve and attend meetings regularly in person and/or virtually
  • Have a positive approach and work in partnership with others
  • Respect diversity and the perspectives of others
  • Adhere to the Hackensack Meridian Code of conduct
  • Complete the Volunteer application process (if applicable)
  • Review and Sign the Patient Family Advisory Council Confidentiality Agreement
  • Participate in activities such as special projects or committees, as needed.
