Hackensack Meridian Health offers a range of primary care services for any age – Family Medicine, Internal Medicine and Geriatric Medicine. You have a primary care team working together to meet your health and wellness needs.
Doctors
Doctors in Internal Medicine, Family Medicine and Geriatrics are leaders of the primary care team. They diagnose and treat illness and chronic medical conditions. They coordinate care with other specialists and members of the primary care team.
- Internal Medicine Physicians: treats adult patients and sometimes adolescents as young as 16-years-old
- Family Medicine Physicians: trained to treat patients of all ages, from infancy through adulthood
- Geriatrics Physicians: specialists in ailments and conditions experienced by patients who are 70-years-old or older.
Physician Assistants and Nurse Practitioners
These advanced practice providers see patients for the diagnosis and treatment of illness and chronic medical conditions, and coordinate care with your doctor, as needed. They have specialized graduate education, training and certification.
Registered Nurses
Registered nurses are graduates of college nursing programs. They provide patient care including wellness screenings. You may receive health care advice and treatment options from a nurse during a phone call or secure MyChart messaging.
Licensed Practical Nurses and Medical Assistants
Licensed practical nurses and medical assistants often start your clinic appointment with health screenings and assist the provider during your appointment.
Medical Receptionists
Medical receptionists greet you when you enter our office. They have extensive knowledge of medical terminology and procedures to help direct your needs to the proper primary care team member.
Patient Access Center
We have enhanced many primary care practices with a dedicated team to handle your appointment requests, prescription refills and general phone calls.
