Geriatric Fellowship Rotations | Hackensack University Medical Center   

Rotations

  • Clinical experiences are provided at various settings, including ambulatory care, acute care, post acute care, long term care, house calls, memory care (Dementia) center and hospice care. 
  • Fellows have various subspecialties observation experiences
  • Longitudinal mentorship program, each fellow is assigned to a faculty member.
  • There is an individualized mentorship program that allows our fellows to get guidance not only for the fellowship but also for the future plans from our seasoned faculty. 

Inpatient/Acute Care

Faculty: Dr. Iofel and Dr. Deland

It is a geriatric consultative service, goal is for fellows to:

  • Understand and able to do comprehensive Geriatric assessment
  • Provide Age Friendly care based on the 5 Ms. 
  • Delirium management
  • Polypharmacy
  • Understand frailty
  • Discuss goals of care, advance directive
  • Understand hazard of hospitalization for older adults
  • Addressing SDOH

Ambulatory Care

 Faculty: Drs. Parulekar, Sarkar, Jain, Kolessa, Wagle, Omer. 

  • Fellows get to see Complex geriatric patients in their continuity clinic, address geriatric syndromes, how to do assessment of patients with multiple comorbidities, learn to provide Age friendly care and opportunity to work with multiple faculties. 
  • Urgent visit for patients to provide value based care in the ambulatory setting, minimizing burden of ED and hospitalization for the patients and their care providers. 
  • Pre-op geriatric consultation, 
  • Work with social worker to address SDOH and psychosocial needs of the patients and care providers. .

House call at Infirmary

Faculty: Dr. Kolessa

  • Understand logistics of a house calls program
  • Understand benefits of house calls for patients and their care providers
  • Provide routine as well as urgent patient centered care in the house calls setting

Dementia Center

Faculty: Dr. Parulekar and Dr. Wagle

Fellows learn:

  • Comprehensive assessment of patients with cognitive concerns
  • Diagnose and manage different types of dementia
  • How to manage dementia related behavior problems and 
  • Address advance care planning goals of care

Long Term and Post acute Care

Faculty: Dr. Kolessa 

  • Providing them with compassionate, appropriate and effective for the treatment of health problems and the promotion of health, 
  • Coordinating transition of care on patients admitted to facility
  • Evaluating appropriateness for care
  • Determining goals of care
  • Running family meetings
  • Participating in IDT meetings

Inpatient hospice (GIP)

Faculty: Dr. Omer and inpatient faculties

Learn about:

  • When patients are transitioned to hospice 
  • Who makes the decision for hospice and 
  • How to manage uncontrolled end of life symptoms

Research and Quality improvement Initiatives

Fellows have a dedicated time for didactics and research, fellows work on their projects for AGS annual meeting and also quality improvement projects.
