Our Hospital
Based on the campus of Jersey Shore University Medical Center, ranked #4 in New Jersey by U.S. News & World Report, K. Hovnanian Children’s Hospital boasts more than 30 pediatric specialties to comprehensively serve all medical needs of children. In addition to our 24-hour care, including a pediatric emergency department staffed by pediatric medical experts, we are the only Level 1 Pediatric Trauma Center in New Jersey.
Inpatient service boasts 30 patient beds; 6 private and 12 semi-private rooms featuring a fun, fresh design inspired by life at the shore. The Pediatric Intensive Care Unit includes 10 private rooms and enables the hospital to provide the most advanced health services for the region’s sickest children. The hospital also offers a Level III Neonatal Intensive Care Unit, Regional Perinatal Center, and Center for Breastfeeding for the care of all birth parents (especially those experiencing high-risk pregnancies), and their babies.

Our goal is to help you get settled and feel comfortable as quickly as possible. On the day you arrive at Hackensack University Medical Center, you will be greeted by an Admissions and Registration receptionist.
Please have with you:
- Current Insurance Card(s): including Medicare and the current month’s Medicaid card.
- Separate Form of Photo Identification: such as your driver’s license, Passport, etc, that shows proof of address as well.
- Copy of Your Advance Directive (Living Will): If you do not have one, we will provide you with information regarding an Advance Directive.
- The Patient at Home Medication List: should be completed and brought with you.
- Any Required Managed Care Referral and/or Pre-certification Forms (if necessary)
- Employer Information for Workers Compensation Claims and Claim Number (if available)
- Automobile Insurance Card for Automobile Accident Cases
- Responsible Party Information (the guarantor- the person responsible for the bill)
- Military Medical Card (if you are a veteran)

Take the Garden State Parkway South to Exit 100A. Follow Route 33 East (also Corlies Avenue) for approximately 3.1 miles. The parking entrance for Jersey Shore University Medical Center will be on the left.
Take Route 35 South to Corlies Avenue (also Route 33) in Neptune. Turn Right onto Corlies Avenue heading West (Route 33) for approximately 1/2 mile. The parking entrance for Jersey Shore University Medical Center will be on the right.
Take Route 18 South to Exit 8a – Route 33 East. Route 33 is also Corlies Avenue. Follow approximately 1/2 mile. The main parking entrance for Jersey Shore University Medical Center will be on the left at the 2nd traffic light.
From the South
Take the Garden State Parkway North to Exit 100A. Follow Route 33 East (also Corlies Avenue) for approximately 3.1 miles. The parking entrance for Jersey Shore University Medical Center will be on the left.
Take Route 35 North to Corlies Avenue (also Route 33) in Neptune. Turn Left onto Corlies Avenue heading West (Route 33) for approximately 1/2 mile. The parking entrance for Jersey Shore University Medical Center will be on the right.
From the East
Follow Corlies Avenue (also Route 33) for approximately 1/2 mile. The parking entrance for Jersey Shore University Medical Center will be on the right.
From the West
Take Route 33 East to the Route 33/34 Circle. Continue East on Route 33 (also Corlies Avenue) for approximately 6 miles. The parking entrance for Jersey Shore University Medical Center will be on the left.
Take Route 195 East to Exit 36. Follow signs to Garden State Parkway North (approximately 1 mile after exit). Take the Garden State Parkway North to Exit 100. Follow Route 33 East (also Corlies Avenue) for approximately 3.1 miles. The parking entrance for Jersey Shore University Medical Center will be on the left.
Directions to our Outpatient Services & the Amdur Ambulatory Care Center
From the East
Take Route 33 West and pass Walgreen’s on your right and make the 2nd right into the Amdur Entrance. This entrance is right before the red light. Please proceed straight for our complementary valet or make an immediate right for surface lot parking.
From the West
Take Route 33 East and make left into the hospitals main entrance (at light) and stay left on the loop road. Follow that around the parameter of the campus following the signs for “Amdur Parking”. You will make a right directly after the parking garage and travel back toward 33. You will see parking spaces throughout this area. You are encouraged to use valet which is complementary. Make a right at the end of the building and then another right to valet.
Your physician will provide you with specific instructions for your procedure including, but not limited to, the date and time of your appointment.
Physician Recommendations
Your physician may recommend that you have risk assessment (medical clearance) by a Hackensack Meridian Health Hackensack University Medical Center physician to determine how well you will tolerate a procedure based on your medical condition.
You need to notify your insurance company
You and your physician must advise your insurance company of your scheduled admission and/or surgical service. Many insurance companies require pre-certification prior to admission date. Please contact your insurance company at least three days prior to the surgery to confirm that the service has been properly pre-certified and that the pre-certification number has been obtained. If it has not been obtained, contact your physicians office and inform them. PROPER PRE-CERTIFICATION WILL ENSURE THAT YOUR SERVICE WILL NOT BE POSTPONED. Medicare and Medicaid patients do not require pre-certification.
Notice of health information practices
If this is your first visit to Hackensack University Medical Center, you will be given a copy of our Notice of Health Information Practices. The notice describes how information about you may be used or disclosed and how you can get access to this information.
Restrictions with anesthesia / sedation
If you have been informed by your physician that you will be given general anesthesia or sedation, you may not operate a car. Please make arrangements for transportation to and from the hospital.
Do not bring any valuables
Jewelry and other valuables should be left at home. If you are staying overnight, you only need personal essentials such as robes, slippers, glasses, dentures, etc.
Pre-admission testing
If the surgeons office instructs you to have pre-admission testing at the medical center, please call to schedule an appointment at 551-996-2099.

What is Registration?
Registration is a process where you check in with us at specific registration desks each time you visit a specific service area within the hospital. Stopping at the Registration Desk ensures that your demographic and insurance information for each appointment is complete and accurate.
Do I have to stop at Registration every time I visit the hospital?
Yes. Depending on the treatment or service you will be receiving will determine if you need to stop at each visit or only once per month. At the initial registration they will inform you. This process enables Hackensack University Medical Center to confirm that both your demographic and insurance information has not changed or , if necessary, update our records if there have been changes since your last visit.
How does Registration work?
At the time of the registration you will meet with a trained Ambulatory Registrar who will review your demographic and insurance information to ensure that the information is accurate. They will also ask for and copy the insurance card and photo identification. At the time they may ask for any out-of-pocket payment that you are responsible for, your prescription, referral and / or any other forms that may be required. After the initial registration any required subsequent registration will be quicker, providing all of the information is available or the same. At the end of the registration the Ambulatory Registrar will advise you to return to the waiting area to wait for your test or clinical visit. Our goal and intention is to expedite the process and ensure that all information is correct to avoid any future billing problems that could be avoided upfront.
Why is Registration necessary?
Accurate registration benefits everyone, including patients/guarantors, providers and insurance companies during this process. Your information in our records is confirmed or updated to ensure accurate claim submissions with your insurance.
What information will I need to bring?
Please bring your current insurance card(s) and insurance address to your appointment. If your visit is related to an accident, worker’s compensation, employment examination or other special purpose, please bring the appropriate coverage information including any pertinent forms your physicians need to complete. Please inform your registrar whether your spouse works or if you have multiple coverages so that we can determine which insurance should be primary.
Do I have to make a payment before I receive services?
You will be asked to make a payment based on your insurance company’s co-pay, deductible, and/or co-insurance. If you have no insurance you will be expected to make an advanced deposit for a non-emergency/elective visit. You will receive a courtesy call 72 hours or more prior to your scheduled visit to provide you with option of paying over the phone to expedite your registration or you may call Financial Clearance for Ambulatory registration at CALL 551-996-4733 to discuss payment plan options or request to speak with a Financial counselor for Healthcare Assistance options if qualified.
What if my visit is work or accident related? Do I still need to stop at Registration?
Yes. Please provide the registrar all requested data regarding your Worker’s Compensation and No-Fault carrier information. Informing Registration that your visit is work or accident related will ensure that your claim is processed in a correct and timely manner.
What if I do not see a doctor but have lab tests or x-rays done instead, do I still need to stop at Registration?
Yes and in addition to the insurance card and photo identification you must bring a prescription from your physician which also includes the diagnosis. Without this we cannot promise you that we will be able to perform the service for you . If your services requires a Pre-cert or referral, please ensure that you provide this data if your ordering physician has not done so.
Will I have to sign anything?
Yes. You may be asked to sign an Out Patient Consent to treat which is valid for one year from the date signed for any outpatient service . This document includes giving Hackensack University Medical Center the authority to release information to your insurance company on your behalf. You will also have to sign a Financial Agreement form where applicable.
Will I receive anything at Registration?
Yes. You will receive the Notice of Patient Privacy statement, the Patient Bill of Rights t(only at your initial visit good for all future services ), and a receipt for any payments made as well as other forms that may be applicable to your visit.
I was just here last week and none of my information changed. Do I still need to register?
Yes. For your safety, but you will be “express registered “ Registering for each visit will help us ensure we have correct, current information in your medical record. It usually takes less than a minute to check in and we may have information for you (routing instructions, for example). This will help speed up your visit and get you to your appointment promptly
Should I arrive early for my visits?
Yes. Please arrive approximately 15-20 minutes prior to your scheduled out-patient appointment. Specific departments will advise you for other arrival times.

Pastoral Care team members at Jersey Shore provide spiritual care to persons of all faiths during their stay, responding to spiritual questions, concerns, and crises just as our medical teams respond to physical needs. We focus upon the patient as a whole person, so that any concerns – emotional, spiritual, or personal – are addressed in a caring, compassionate manner.
The Pastoral Care Team consists of a number of professional chaplains and caregivers who deliver pastoral care to ensure that a variety of religious and spiritual needs will be met for all patients, families, and staff. Rev. David Cotton is the leader responsible for all the activities of the team members as well as providing direct ministry to patients, families, and Jersey Shore team members. Our staff also includes a full time Roman Catholic Chaplain, chaplains who minister to persons of all faiths, and an on-call Rabbi. The professional team is supported by a large group of dedicated Eucharistic Ministers, pastoral care volunteers, and local community clergy of all faiths.
Jersey Shore welcomes persons of all faiths to pray, meditate, or just spend some quiet time in our beautiful Young Interfaith Chapel, located on the first floor of the Booker Pavilion. For our Catholic patients, families, and staff, Mass is said Tuesday through Friday at 12 p.m. in the chapel and broadcast over closed circuit television, channel 48.

The pharmacy is available to patients to fill any necessary prescriptions before they even leave the hospital’s doors. The Hackensack Meridian Health Pharmacy accepts most prescription plans, and payment can be made by cash, check, or credit card.
The Pharmacy also offers two dedicated parking spaces right in front of the hospital’s main entrance.

Concierge Care has made arrangements with local restaurants to deliver meals for patients and their guests. The menus have been customized to include important nutritional information.
